I agree to release Beth Tikvah Synagogue, its employees, directors and officers, other floor hockey participants, and all persons involved directly or indirectly in the administration of Beth Tikvah Synagogue Men's Floor Hockey from liability and all claims for injury, loss, death or damage to person or property, howsoever caused, arising during the course of any floor hockey game or practice or any other event associated with men's floor hockey at Beth Tikvah Synagogue.
